MONTHLY SUPERVISION REPORTS

For Pretrial Defendants and Post-Conviction Offenders

Standard Condition #2 of the Conditions of Probation and Supervised Release requires that “The defendant shall report to the probation officer and shall submit a truthful and complete written report within the first five (5) days of each month.”

The Monthly Supervision Report (MSR) must be completed fully, front and back, with all requested information, including vehicles used as well as income and employment information. It must include your current address and telephone numbers, such as cell phone numbers. It must be signed and dated. You are also required to provide copies of your paystubs as proof of monthly income as well.

It is recommended that you mail the Monthly Supervision Report to the Probation Office where the assigned U.S. Probation Officer is located, as long as it is received in the office by the 5th of each month. You may be required to report at other times during the month as directed by your U.S. Probation Officer.
